Problems with the joint structure itself, known as osteoarthritis, can also damage the cartilage, as can being overweight for a lengthy period. You can also experience cartilage damage if you are bedridden or other wise immobile for long periods, because the cartilage needs regular movement to function correctly. This is connected with the blood supply, which will be discussed shortly.

Cartilage is constructed of cells known as chondrocytes that generate a fibrous matrix known as collagen, a mixture of amino acids known as elastin that allows the cartilage to return to its original shape after deformation, and non-collagenous matrix tissue containing proteins, water and proteoglycans that contain sulfated glycosaminoglycan chains. That last mixture is often referred to as ‘ground substance.’

One of the problems with cartilage is its lack of a direct blood supply, and it relies on the compression and decompression of the articular cartilage, or on the flexing of elastic cartilage, to create a pumping action that drives blood to the chondrocytes. This is why inactivity can cause cartilage damage, due to a lack of blood supply, and why it is repaired more slowly than other body components.

Once an injury or wear and tear damages a joint, the body’s natural defense, the immune system, is activated, and the major part of that involved in cartilage damage is the inflammatory response. The joint becomes inflamed, the quantity of synovial fluid is increased to provide more protection and swells the joint, and enzymes (hyaluronidase) are produced which, although part of the natural defense system, actually degrade the synovial fluid and the cartilage.

This increases the amount of inflammation and the process becomes self-perpetuating, leading to the condition known as degenerative joint disease (DJD) because the body is unable to produce enough glucosamine to generate the proteoglycan needed for repair.

This is where glucosamine sulfate enters the scene. Glucosamine is a precursor for glycosaminoglycans (GAG), which as mentioned as above are components of proteoglycans in the cartilage matrix ground tissue. It has been shown to stimulate the biosynthesis of proteoglycan, and analysis has shown its presence within articular cartilage after administering it orally to patients with cartilage disease. It therefore makes its way to the right place.

Glucosamine is administered in the form of glucosamine sulfate, the highly electrically charged sulfate groups believed to aid in the compression properties of cartilage. It is rapidly absorbed into the bloodstream, although only about a quarter of the oral dose is eventually available to the body, and high concentrations accumulate in the liver, kidneys and in articular cartilage where it is used in the biosynthesis of GAG.

When in solution, glucosamine sulfate separates into ions: sulfate and glucosamine. Glucosamine ions are involved in the synthesis of GAG, that then combine with proteins to form proteoglycans, a component of the non-collagenous matrix of the cartilage. Although glucosamine is the major active component, there is evidence that the sulfate group contributes the stability of the matrix of the connective tissue since the uptake of sulfate ions increases with the amount of glucosamine sulfate used.

Another consideration here is that sulfate is an important part of proteoglycans, and glucosamine sulfate promotes not only the synthesis of glycosaminoglycans, but also of proteoglycans in general. Glucosamine is also active in regenerating the lubricating properties of the synovial fluid, and in hindering the activity of hyaluronidase, the enzyme that breaks down the hyaluronic acid in the synovial fluid.

Some people find that glucosamine, taken either alone or in conjunction with chondroitin sulfate and/or methyl sulfonyl methane (MSM), is more effective than the non-steroidal anti-inflammatory drugs (NSAIDs) used to reduce inflammation (e.g. Aspirin and Ibuprofen) and without the side effects of these substances. MSM contains dietary sulfur, which is necessary for cell structure and healthy cell repair. Methyl sulfone methane is know to be beneficial for painful conditions such as arthritis, and also improves the blood circulation. It might also play a part in helping glucosamine sulfate get to the site of the cartilage damage.

Glucosamine is a large molecule, however, and finds it difficult to make its way to the area around the joint due to the lack of a direct blood supply. Ginkgo biloba, also known as the maidenhair tree, grows naturally in two small areas of Eastern China. It is believed that even these sources are artificial, and planted and maintained by monks over many centuries, and that there are no true natural sources of the tree left. Other than these, all living ginkgo trees are now artificially farmed.

The ginkgo seeds contain nuts that are a traditional food in China, served at Chinese New Year, and on other special occasions such as weddings, and is also used in traditional Chinese medicine. The seeds have been used in the treatment of coughs and asthma, and during the late 1970s and 80s, the uses of ginkgo biloba in medicine was extensively investigated with a view to determine the range of conditions that the tree could be used to treat. Given that many ancient Chinese remedies have found to be effective, and with a relevant scientific basis, this was a logical step.

It has been established that ginkgo biloba could have three possible effects on the body. These are:

* Improvement in circulation including that in the small capillaries.
* An antioxidant effect against free radicals.
* Prevention of some of the harmful effects of blood clotting and aggregation of platelets.

The last of these has been responsible for many cardiovascular conditions, and disorders of the kidneys, lungs and central nervous system, and is due to the effect of the platelet-activating factor (PAF) that ginkgo appears to inhibit.

Before delving deeper into the possible beneficial effects of Ginkgo, let us first examine the active ingredients believed to be involved in the perceived beneficial effects.

Ginkgo leaf extract contains terpenoids (bilobalides and ginkgolides) and flavonoid glycosides. Flavones can reduce the fragility of capillaries, and protect the body from blood loss through damaged capillaries, particularly in the brain. The Ginkgolides, particularly ginkgolide B, inhibit the platelet-activating factor and so increase the fluidity of the blood that improves circulation, again particularly in the micro-capillaries of the brain. This is also why it is believed to reduce the incidence of cerebral thrombosis and resultant strokes.

The antioxidant effect of ginkgo biloba extract (GBE) has been widely studied, and by scavenging free radicals the extract can help to prevent cell membrane damage, and so prevent cells from being destroyed. It has been established that pretreatment of cells with GBE can prevent such damage in rats.

The anti-inflammatory properties of ginkgo biloba, as seen in some asthma patients for example, is thought to be due to the inhibition of the platelet-activating factor (PAF) by ginkgolide B, PAF playing a significant part in the inflammatory response to allergens, and PAF is now believed to be responsible for conditions such as asthma, renal diseases, central nervous system disorders and ischemia, a restriction in the blood supply, particularly to the brain.

It follows, therefore, that the effect of GBE on these conditions is likely to be due to PAF inhibition, and a reduction in the inflammatory response to a number of conditions. What evidence is there for this? In fact, results of the trials carried out have been inconclusive one way or the other.

Hence, a trial published by the Journal of the American Medical Association reported no effects after a 6 week trial of Ginkgo on Alzheimer’s and memory disorders. However, other trials have indicated positive effects after 6 weeks, and it could be that the GBE has to be taken for more than 6 weeks for any effects to be noticed. It is certainly believed to be a longer term treatment rather than have instant results, although some tests have shown improvement in concentration for up to 2.5 hours after treatment. The bulk of the evidence is favorable on the effect of GBE on memory disorders.

Test on rats, in which the blood flow to the brain was mechanically blocked by carotid compression, indicated that ginkgo biloba promoted an increase in the glucose and ATP levels in the brain neurons. Other trials have indicated that neurological damage in mice subjected to neurotoxins was reduced by the administration of GBE, and while not conclusive with respect to humans, the effect of GBE on the brain appears to be more than just opinion.

To sum up, ginkgo biloba is believed to be effective in treating the following disorders by virtue of its effect in improving the fluidity of the blood, protecting fragile capillaries from damage, exerting an antioxidant effect on free radices, and so prevent damage to cell membranes, and its inhibiting effect on platelet-activating factor:

Circulation Problems

Circulation problems in the arteries can lead to blood clots that in turn cause strokes and cardiac problems. By preventing blood clots through the inhibition of PAF, ginkgolide B can help to maintain a healthy circulation system that also help to maintain circulation in the very small capillaries that feed the brain.

Q. What is CoQ10?

A. CoQ10 is a natural, fat-soluble nutrient present in virtually all cells. CoQ10 also is known as ubiquinone (existing everywhere there is human life). This scale is known as the Unified Parkinson’s Disease Rating Scale (UPDRS). This process is known as establishing “baseline values,” that is, the condition of the patient before receiving any treatment.

Participants were divided into 4 groups. Each group received either 300 mg, 600 mg, or 1200 mg of the special form of CoQ10, or a placebo. The researchers observed the participants for 16 months.

The results of the study showed that all the participants who received CoQ10 had smaller declines in function compared to the placebo group, but the smallest decline was experienced by the group taking the highest amount of the special form of CoQ10.

The most significant results were noted specifically in the activities of daily living scores by the people taking 1200 mg of CoQ10 daily. These people retained better ability to feed and dress themselves, speak, walk, and bathe or shower by themselves. They maintained greater independence for a longer time. Parkinson’s disease, as with other neurodegenerative diseases, robs the sufferer of their ability to control the movements of their own body and care for themselves. Supplementation with CoQ10, while not a cure, is the first intervention that showed a slowing in the progressive deterioration of the function associated with this disease.

Q. What were the results of clinical research on Huntington’s Disease?

A. A randomized, double-blind, placebo controlled study respected type of study, was conducted at the University of Rochester. All of the 347 Huntington’s disease (HD) patients were experiencing some HD symptoms, but were still in the early stages of the disease. The patients (who did not know which drug they were receiving) were randomly assigned to four different treatment groups: 25 percent received Remacemide, 25 percent received CoQ10, 25 percent received both, and 25 percent received a placebo, or sugar pill. The researchers, who also did not know which patients got which drug, watched and recorded their progress for two and one-half years. Remacemide is a new drug made by Astra Seneca that blocks the neurotransmitter glutamate in the brain, that has long been suspected of contributing to the death of brain cells in Huntington’s disease.

Unfortunately, in the CARE-HD study, Remacemide had no effect on the progression of the disease in patients in the early stages. However, the individuals who received 600 mg of CoQ10 per day experienced some slowing of the disease progression. They were able to manage daily activities, such as meal preparation, housekeeping tasks, and personal care longer than those not on CoQ10. They were also able to focus their attention better and were less depressed and irritable. The portion of the studied patients receiving 600 mg of CoQ10 per day experienced a 15 percent decline in the progression of HD. According to the researchers conducting the study, a 15 percent decline in the progression of HD would roughly translate into approximately one more year of independence for patients. This is the very first study from more than a dozen Huntington’s disease patient trails that showed any modification of the course of the illness.

Of note, the effects of the CoQ10 had not abated at the end of the research study. That is, the benefit of using CoQ10, 600 mg per day, was still increasing; this suggests that the longer a patient supplements with CoQ10, the greater the decline in the progression of HD. The next phase of the CARE-HD research will test a higher dose of CoQ10 (1200 mg or more per day), with more patients (over 1000), for a longer period of time (approximately 5 years). This study should improve our understanding of the optimal dose and the total achievable decline in the progression of HD. The CoQ10 product used in the CARE-HD study was designated an Orphan Drug by the FDA. A. Studies show CoQ10 levels are greatly reduced in Alzheimer’s patients. Mitochondrial abnormalities also are noted; however, research has yet to determine how or why this occurs. Some scientists believe damage to mitochondria is an early feature of the disease. Free-radical damage also is a feature of Alzheimer’s.

In a study of 27 Alzheimer’s patients, subjects were given 60 mg of CoQ10, 150 mg of iron, and 180 mg of vitamin B6 daily. Each patient’s mitochondria activity was effectively activated. All patients continued to experience gradual decline. However, researchers believed that with this combination, the progression was much slower and allowed the patients to experience 1 to 2 years of extended good health.

ALS (also known as Lou Gehrig’s disease) is a progressive, fatal, neurological disease. It occurs when the nerve cells in the brain that control voluntary movement gradually degenerate. Investigation of CoQ10 in individuals with ALS is just beginning. Researchers at the Eleanor and Lou Gehrig ALS Center at Columbia University recently conducted a small clinical pilot trial of CoQ10 in ALS. The study was an open label study, which meant that everyone enrolled received CoQ10, 400 mg three times per day. Of the 16 patients originally enrolled, nine patients completed the study. Six of these nine patients experienced some benefits. The patients declined from 0 – 25 percent in functional scores, 6 percent in strength, and 10 percent in breathing ability. These scores reflect a positive trend compared to the 50 percent decline that is seen in the natural history of ALS over the same period of time (5 to 9 months). Citing the need to conduct more studies of the effectiveness of CoQ10 for people with ALS is rapidly and efficiently as possible to get answers to patients and clinicians, another clinical trial is currently underway at the Gehrig ALS Center. This is a pilot study to determine if CoQ10 has short-term effects on motor nerves in the brain using magnetic resonance spectroscopy (MRS). The researchers are going to try to “see” if CoQ10 can change the chemical sin the brain’s upper motor nerves of people with ALS, an important next step of the investigation.

Q. Can taking CoQ10 prevent neurodegenerative disease?

A. To date, there have been no studies or research examining whether CoQ10 can prevent these diseases.

Alzheimer’s disease prevention is being clinically investigated. Researchers have determined that people who take certain anti-inflammatory medications seem less likely to develop the illness. A large, multi-centered trial is studying this connection.

Q. How much CoQ10 should I take?

A. Depending on your family history of neurological disease and your disease experience, studies show benefits at doses of 100 to 200 mg of CoQ10 daily. Some studies used doses of up to 1,200 mg per day.

CoQ10’s safety has been evaluated. To date, no toxicities have been reported. Mild stomach upset may occur. Taking CoQ10 with meals usually alleviates this rare effect.

    For a sea cure bath, mix together half a pound of sea salt and a pound of baking soda, add to a warm water bath and soak until the water has cooled, says Mok. "It's excellent for soothing itchy and dry skin and helps detoxify by pulling out toxic waste from the pores." Aromatherapy oils, like lavender, make your soak in the tub even more relaxing and luxurious. "It's a great way to de-stress after a long day at work."

    A seaweed wrap can release water retention and leave legs looking their sleekest, notes Mok. "Just soak legs in a bath of warm water and Epsom salts for 5 minutes, then pat dry. Apply a seaweed mask and wrap legs with plastic wrap and a warm towel. Relax for 15 minutes. Remove towel and plastic wrap and rinse."

    You can also try a sea salt rub by mixing two cups of kosher salt with one cup of olive oil until it forms a thick paste. (Be careful: the oil is slippery.) "While in the tub or shower, massage it into your skin using long strokes toward the heart, starting with your feet," says Galvez. Rinse off with warm water, use a soft washcloth to remove any residue, pat dry and apply moisturizer. "Your skin will be silky smooth and wonderfully hydrated." To create a spa environment at home, details make all the difference. "Think of your favorite beach get-a-way and go with an ocean theme," says Cox. "Include something for each of the senses." For example, put on a CD that has nature sounds. To capture the color of the water, use sea-colored towels. For scent, light candles that produce the scents of flowering plants (such as plumeria or citrus). Add "ocean" fragrance beads. When taking a bath, "use shells to scoop out sea salts or dehydrated sea weed and put them around the tub as decoration," says Cox. Smooth on a moisturizer with a sea-scented lotion when you finish your spa treatment.

    When you make an at-home sea spa experience a regular part of your routine, you reap a bounty of beauty and health benefits. "In just 20 minutes you can have a mini-vacation," says Galvez. "It's cleansing and relaxing."

    Garlic has been used medicinally for thousands of years. Past research has established the significance of garlic and its healthful benefits. Recently new metabolic roles for garlic were presented at the symposium, “Significance of Garlic and its Constituents in Cancer and Cardiovascular Disease” which was held at the Georgetown University Conference Center on April 9-11, 2005.

    The conference brought together the world’s top authorities in various fields of garlic research to provide the latest updates, specifically regarding aged garlic extract and its actions in diseased states such as cancer.

    One of the highlight presentations was on the biggest study ever conducted in the history of garlic supplementation, a 7 year long clinical study. The study was undertaken to determine aged garlic extract effects on stomach cancer and serous digestive problems that can lead to gastric cancers. It was performed through collaboration of National Cancer Institute (NCI) and leading Chinese researchers, who tested to see if aged garlic supplementation would significantly effect or reduce precancerous gastric lesions. The clinical study was double-blinded and randomized, with over 3,000 human subjects. It was led by Dr. Mitchell Gail at NCI and Dr. Wei-Cheng You at the Beijing Institute for Cancer Research.

    Preliminary results of this study showed that people with the highest intake of allium-containing vegetables, like aged garlic, had only 40% of the risk of gastric cancers as those who rarely ate them.

    Also from the Chinese study was another surprising finding: when researchers used aged garlic extract in combination with antibiotics to treat people with precancerous stomach lesions caused by the Helicobacter pylori bacteria, 76% were completely healed. “It’s a very large and long-term study. We’re still sifting though the data, but we expect to report on it by the end of this year,” said Dr. Gail.

    Another surprising presentation showed the results of several colon cancer studies reported from both Osaka and Hiroshima University in Japan. These studies clearly showed that the addition of aged garlic extract to cancer treatment was beneficial to colon cancer patients. Researchers found that those who consumed aged garlic extract experienced a reduction of the size and prevalence of tumors.

    “Aged garlic extract has well-known antioxidant properties. The aging process eliminates garlic breath as well as those harmful properties, leaving only the antioxidants that protect against disease,” said cancer symposium chairperson and lead UCLA researcher David Heber, M.D.

The symposium was held at the Georgetown University Conference Center on April 9-11, 2005.

    Effect of aged garlic extract (AGE) has been tested in the placebo-controlled double blind randomized clinical study that determined that the atherosclerotic plaque burden detected by electron beam tomography (EBT) changed significantly with the use of aged garlic extract, Patients in Dr. Budoff’s study were able to significantly lower their total cholesterol, blood pressure, homocysteine and LDL cholesterol oxidation levels with aged garlic extract supplementation.

  • • Influence of Garlic on Endothelial Dysfunction in Hyperhomocysteinemia, presented by Norbert Weiss, University of Munich in Germany Aged Garlic Extract (AGE) minimizes intracellular oxidant stress and stimulates NO generation in endothelial cells. Preliminary results show that pretreatment with AGE for six weeks diminishes the adverse effects of acute high homocysteine on endothelium-dependent brachial artery vasodilatation and on acetylcholine-induced stimulation of skin perfusion.

  • • Anti-glycation properties of aged garlic extract: possible role in prevention of diabetic complications, presented by Nessar Ahmed, Manchester Metropolitan University in England Aged garlic extract inhibited the formation of advanced glycation end products, which have been previously shown to increase the risk of diabetic complications ranging from heart disease to retinopathy, kidney failure, impaired wound healing and many more.

    “Garlic is turning out to be a major player in cancer and heart disease prevention and control, especially in combination with drug treatments,” said Richard Rivlin, M.D. of Strang Cancer Prevention Center at Cornell. “It’s also showing us that we can start early. It’s madness to treat cancer and heart disease in their advanced stages. We need to start early and aged garlic extract is an excellent way to do that.”

    PRECLINICAL PERSPECTIVE ON GARLIC AND CANCER. John A. Milner, National Cancer Institute, Rockville, MD 20892

    Mounting evidence points to the anticancer properties of fresh garlic extracts, aged garlic, garlic oil, and a number of specific organosulfur compounds from garlic. These prevention characteristics arise through both a dose and temporal related change in several cellular events including those involving drug metabolism, immunocompetence, cell cycle regulation, apoptosis and angiogenesis. A block in carcinogen activation through modulation of cytochrome P450-dependent monooxygenases and/or acceleration of carcinogen detoxification via induction of phase II enzymes likely account for some of this protection. The block in preneoplastic lesions and/or tumors in several sites suggests a generalizable mechanism. The efficacy of water- and lipid-soluble allyl sulfur compounds against chemical carcinogenesis appears comparable, although more studies are needed. A shift in sulfhydryl groups, redox status or enzyme catalysis may account for some of the phenotypic changes. They may also account for the observed hyperphosphorylation of specific cell cycle related proteins and histone hyperacetylation; both of which have been correlated with suppressed tumor cell proliferation. Several forms of allyl sulfur compounds are effective in blocking cell division and inducing apoptosis, but notable differences in the efficacy among these various compounds and across tumor types are evident. While the expression of many genes and proteins can be influenced by allyl sulfides; the challenge is to determine which is responsible for a phenotypic change. Additional studies are needed with more modest exposures and over prolonged periods and that utilize transgenic and knockout models to assist in the identification of molecular targets. Finally, additional research is needed to identify sensitive “effect” and “susceptibility” biomarkers that can ultimately be used to identify responders from non-responders.

    INHIBITION OF CORONARY ARTERIAL PLAQUE ACCUMULATION BY GARLIC. Matthew Budoff, Harbor-UCLA Medical Center, UCLA School of Medicine, California, USA

    Effect of Aged garlic extract (AGE) has been tested in the placebo-controlled double blind randomized clinical study to determine whether the atherosclerotic plaque burden detected by electron beam tomography (EBT) will change at a different rate under the influence of AGE or placebo. EBT can non-invasively quantitate the amount of coronary calcification and track atherosclerotic plaque over time. Nineteen of 23 patients completed the study protocol. The patients were well matched for age, gender, statin use and cardiac risk factors. Patients underwent EBT and blood testing at baseline, and then again after 12 months of randomization. The average change in the calcium score (Volumetric method) ± SD for the AGE group (n = 9) was 7.5 ± 9.4% over the one year. The placebo group (n = 10) demonstrated 22.2 ± 18.5% annual progression, significantly greater than the treated cohort (p = 0.01). While there were no significant changes in cholesterol parameters, or C Reactive protein between the groups, high density lipoproteins and plasma homocysteine in the AGE group demonstrated a trend toward improvement compared to the placebo patients. Thus, although this is a small-scale trial, it demonstrates the potential of AGE to inhibit the rate of atherosclerosis (progression of coronary calcium), as compared to placebo over one year. Larger studies need to be performed to assess this potential anti-atherosclerotic therapy and the impact on coronary events.

    Endothelial dysfunction (ED) due to decreased bioavailable nitric oxide (NO) by increased vascular oxidant stress plays a critical role in the vascular pathobiology of hyperhomocysteinemia (hhcy). Aged Garlic Extract (AGE) minimizes intracellular oxidant stress and stimulates NO generation in endothelial cells. We performed a placebo-controlled, blinded, cross-over study to examine whether AGE prevents macro- and microvascular ED during acute hhcy induced by an oral methionine challenge in healthy subjects. Acute hhcy leads to a significant decrease in flow-mediated vasodilation of the brachial artery as determined by vascular ultrasound, indicative of macrovascular ED, as well as a decreased number of recruited nailfold capillaries during postischemic reactive hyperemia as determined by videomicroscopy, and to a decreased ratio of acetylcholine (endothelium-dependent) vs. sodium nitroprusside (endothelium-independent) iontophoresis induced skin perfusion as measured by laser doppler flowmetry, indicative of microvascular ED. Preliminary results show that pretreatment with AGE for six weeks diminishes the adverse effects of acute hhcy on endothelium-dependent brachial artery vasodilation and on acetylcholine-induced stimulation of skin perfusion. Whether or not this is accompanied by changes in biochemical parameters of ED is still under investigation. It is concluded that AGE may at least partly prevent a decrease in bioavailable NO during acute hhcy.
